Job summary

Conviva Primary Care, part of Humana Inc., seeks a primary care physician for the Orange City, FL clinic. You will report to the Associate Medical Director and work onsite.

Clinics run Monday–Friday with about 15 patients per day; focus on longer, value-based care appointments and on-site work in Orange City, FL.

Qualifications

• Board certified or board eligible in geriatrics, family medicine or internal medicine.
• Valid Florida medical license (or eligible to obtain one).

• Two+ years in senior value-based care.
• 1+ year managing Medicare Advantage panel.

Responsibilities

• Provide primary care services at the Orange City clinic.
• Report to the Associate Medical Director.
• Onsite work at Orange City, FL clinic; participate in patient care across the day.

Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic in Orange City, FL.

As a Primary Care Physician, you will report to the Associate Medical director. You will be onsite at the Orange City, FL clinic.

A day in the life of a Conviva physician

• Monday-Friday 8:00am-5:00pm

• See an average of 15 patients per day

• Longer authorized appointments

• You will be on-site located in Orange City FL

Required Qualifications

• Doctor of Medicine or Osteopathic Medicine

• Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine

• Valid, unrestricted license in the state of Florida (or eligible to obtain one)

• Current, unrestricted DEA license

• Screened for TB

• Minimum of two+ years of experience in senior value base care

• 1+ years of experience managing Medicare Advantage panel of patients

• Associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government. #LI-FM1

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$255,000 - $298,500 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
