Job Profile Summary

Responsibilities
  • Accountability for outcomes: Demonstrate accountability for outcomes, strong clinical care, and cost‑effectiveness for each patient in up to a 450‑patient panel.
  • Health coaching: Act as a health coach, help patients set short and long‑term health goals, partner with patients to work toward goals, and frequently follow up.
  • Simplifying for action: Simplify and prioritize appropriately so that behavior change is more actionable for patients and for care teams.
  • Work independently and collaborate with other health‑care professionals to provide care for patients with acute and chronic illnesses in older adults.
  • Consult specialists as needed and remain responsible for detailed care of patients, including assessment, diagnosis, treatment, management, education, health promotion, care coordination and documentation.
  • Engage with hospitalists, transitional care teams, case managers, and other resources for hospitalized, SNF, LTC or homebound patients.
  • Lead a care team that includes a care promoter (medical assistant), care facilitator, and care coordinator.
  • Participate in risk and quality management programs, clinical meetings, and other meetings to promote patient health and company goals.
  • Adhere to departmental goals, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
  • Support center management, cover other providers, recruit patients and additional providers, and supervise or collaborate with advanced practice practitioners.
Knowledge, Skills & Abilities
  • Availability and accessibility for patients: Open for walk‑in visits and respond to phone calls and messages in a timely manner.
  • Service orientation: Provide care that patients would want for a family member or themselves at every interaction.
  • Evidence‑based medicine: Stay updated on evidence‑based practices and recognize the impact of lifestyle and nutrition on patient outcomes.
  • Physician leadership: Continuously develop leadership skills for patients, team, center and company.
  • Quality focus: Work with care teams to achieve high‑quality outcomes and quality measures and continuously improve practice and operations.
  • Influence: Competently and compassionately influence patients, teams and self to achieve best outcomes.
  • Self‑care: Maintain physical, mental, social, and spiritual well‑being to provide best care.
  • Proficiency in Microsoft Office Suite and other common software.
  • Ability and willingness to travel locally, regionally and nationally as needed.
  • Fluency in English (written and spoken). Fluency in other languages may be required for some centers.
  • Use of independent judgment.
Education and Experience Criteria

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. Must hold a current State Medical License and be able to obtain one for the state(s) in which they will work. Board certification in the specialty is preferred; board eligibility is required. Must maintain board certification through MOC, CME, or retesting as required. Current DEA number for schedule II‑V controlled substances. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within first 90 days of employment.

Pay Range

$214,700 – $306,714 Salary.
