Primary Care Physician (PCP) Demonstrates
  • Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. They influence patient outcomes by building trust and helping patients change behaviors.
  • Coaching for health: The PCP acts as a health coach, helping patients set short‑ and long‑term health goals, partnering with them to work toward those goals, and frequently following up on progress to improve health.
  • Simplifying for action: The PCP prioritizes appropriately so that behavior change is more actionable for patients and for the care team.

We are an outcomes-focused, value-based organization. Metrics for each PCP’s panel of patients include patient admissions per thousand, use of between 18‑21 appointment slots per day (each new patient counts for 2 slots, follow‑up patients 1 slot), CGCAHPS, clinical gaps closures, and medical cost measures. Each PCP will have goals for these metrics and is expected to work toward them with center and market leadership and the care team.

Essential Job Duties / Responsibilities

The PCP independently provides care for patients with acute and chronic illnesses encountered in older adults. The PCP takes full accountability for patient care and outcomes, seeks consultation from specialists when needed, and remains involved in detailed care. The PCP engages with the hospitalist whenever a patient is hospitalized. The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.

The PCP leads a care team that includes a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office. For patients unable to come to the office—hospital, SNF, LTC, or homebound—the PCP engages the transitional care team and other resources available in the market.

The PCP actively participates in center management, covers for other providers as needed, assists in recruiting patients for the center and additional providers, and works with an Advanced Practice Practitioner while being accountable for the APP’s actions.

  • Influence: The PCP competently influences patients, teams, and themselves to achieve the best outcomes.
  • Self‑care: The PCP maintains personal well‑being to provide the best care for patients.
  • Technical proficiency: The PCP is proficient in Microsoft Office Suite and other relevant software used in the company.
  • Travel ability: The PCP can travel locally, nationally for initial training, and regionally or nationally for recruiting or training purposes as needed.
  • Language fluency: The PCP must be fluent in English; other language fluency may be required in some centers and will be specified.
Education and Experience Criteria
  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
  • State medical license (current, active) for the state(s) of employment.
  • Board certification in the specialty preferred; board eligibility required. After certification, the PCP maintains certification through MOC/CME or re‑examination.
  • Current DEA number for schedule II‑V controlled substances.
  • Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross required within first 90 days of employment.
Pay Range

$214,700 – $306,714 salary
