Job summary

The Primary Care Physician (PCP) demonstrates accountability, coaching, and simplification for action. They manage a panel of up to 450 patients, provide cost‑effective care, cultivate trusting relationships, and lead care teams toward performance goals.

Key Responsibilities

• The PCP independently provides care for patients with acute and chronic illnesses encountered in older adults.
• The PCP takes full accountability for patient care and outcomes and appropriately seeks consultation from specialists when needed.
• The PCP engages with hospitalists whenever a patient is in the hospital, regardless of hospitalist affiliation.
• The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.
• The PCP leads their care team, which includes a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
• For patients unable to come to the office—hospital, SNF, LTC, or homebound—the PCP engages with the transitional care team and other resources available.
• The PCP has an active role in the management of their center and covers for other providers as needed.
• The PCP takes an active role in recruiting patients for the center and additional providers for the company.
• The PCP supervises or collaborates with an Advanced Practice Practitioner (APP) and remains accountable for the APP while employed with the company.
• The PCP performs other duties as assigned and modified at the manager’s discretion.

• Availability and accessibility: The PCP is available to patients by walk‑in visits and by promptly answering phone calls and messages.
• Service orientation: The PCP provides care they would want for a family member or themselves to each patient.
• Evidence‑based medicine: The PCP remains updated on evidence‑based medicine and recognizes that lifestyle and nutrition impact patient outcomes.
• Leadership: The PCP continuously develops leadership skills for the benefit of patients, teams, center, and company.
• Quality: The PCP works with care teams toward high‑quality outcomes and measures.
• Influence: The PCP competently and compassionately influences patients, teams, and themselves to achieve best outcomes.
• Self‑care: The PCP takes care of themselves physically, mentally, socially, and spiritually to provide best care.
• Proficient in Microsoft Office Suite and other software used in the company.
• Willingness to travel locally as needed, and nationally for initial training (2 weeks) and sometimes regionally or nationally for recruiting or training.
• Fluency in English, verbal and written. Additional languages may be required depending on the center.
• Use of independent judgment and flexibility to work evenings, weekends, or holidays as needed.

Education and Experience Requirements

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
• Current State Medical License for the state(s) in which the PCP will be working.
• Board certification in the specialty preferred; board eligibility required. MOC, CME, and retaking exams as necessary.
• Current DEA number for ScheduleII–V controlled substances.
• Basic Life Support (BLS) certification from the American Heart Association or the American Red Cross required within the first 90 days of employment.

Compensation

• Pay range: $221,141 – $315,915 per year.
• Compensation may include bonuses or commissions.
