Job summary

Humana Inc. through CenterWell Senior Primary Care seeks a primary care physician to provide comprehensive outpatient care at the Leesburg, FL clinic, Monday–Friday 8:00am–5:00pm.

You will work with an interdisciplinary team in a senior-focused primary care setting under the associate medical director. The role requires MD/DO with board certification or eligibility in family medicine, internal medicine, or geriatric medicine, a Florida license, DEA license, and 2+ years of relevant experience.

Qualifications

• MD/DO board certification or eligibility in family medicine, internal medicine, or geriatric medicine.
• Valid, unrestricted Florida license (or willingness to obtain).
• Current unrestricted DEA license (or ability to obtain).
• At least two years of experience in primary care, geriatrics, or chronic disease management.

Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Collaborate with interdisciplinary teams to ensure holistic care.

Education

MD/DO board certified or board eligible in family medicine, internal medicine, or geriatric medicine

Valid license in Florida (or willing to obtain)

Current unrestricted DEA license

Required Qualifications

• Doctor of Medicine or Osteopathic Medicine Board certification or board eligible in family medicine, internal medicine, or geriatric medicine
• Valid, unrestricted license in the state of Florida (or willing to obtain)
• Current unrestricted DEA license, or must obtain
• Two years or more experience in primary care, geriatric care, or managing chronic conditions

If selected for this role, you will be required to be screened for TB.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$255,000 - $298,500 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
