Job summary

Conviva Primary Care in Lakeland, FL is hiring a primary care physician to join our full-time team on-site. You will provide comprehensive outpatient care to adults and seniors, with a focus on value-based, patient-centered outcomes.

You will be part of a collaborative, interdisciplinary team within Humana's Primary Care Organization, reporting to the associate medical director. A Florida license and board certification (or eligibility) are required or in progress.

Qualifications

• MD or DO from an accredited medical school.
• Active Florida medical license or ability to obtain one.
• Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine (or board eligible).
• DEA license or ability to obtain one.
• One year+ experience in value-based health care.
• TB screening required if selected.
• Willingness to participate in on-call coverage by phone.

Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Work with interdisciplinary teams to ensure holistic care.

• If selected for this role, you will be required to be screened for TB
• Must be willing to participate in on-call coverage by phone

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$255,000 - $298,500 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
