Job summary

Conviva Senior Primary Care in DeLand, FL, part of Humana, is seeking a Primary Care Physician to join our on-site clinic. You will report to the Associate Medical Director and provide proactive, comprehensive care to Medicare Advantage patients in a senior-focused setting.

The role features a 40-hour workweek, with 8:00 am–5:00 pm hours, about 15 patients daily, and longer 45-minute visits to support complex needs.

Qualifications

• Doctor of Medicine or Osteopathic Medicine required.
• Board certified or board eligible in geriatrics, family medicine, or internal medicine.
• Valid, unrestricted Florida medical license, or ability to obtain one.
• Current, unrestricted DEA license.
• Two+ years of clinical experience.
• Experience managing a Medicare Advantage panel.
• Language proficiency assessment required for non-English speaking associates (ILR test).

Responsibilities

• See an average of 15 patients per day.
• 40-hour work week, Monday–Friday, 8:00 am–5:00 pm.

Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic in DeLand, FL.

As a Primary Care Physician, you will report to the Associate Medical Director. You will be on-site at the DeLand clinic.

A day in the life of a Conviva physician

• Monday-Friday 8:00 am-5:00 pm
• See an average of 15 patients per day
• Longer authorized appointments (45 minutes)
• You will be on-site

Required Qualifications

• Doctor of Medicine or Osteopathic Medicine
• Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
• Valid, unrestricted license in the state of Florida, or able to obtain one.
• Current, unrestricted DEA license
• Screened for TB
• Minimum of two years of experience
• One or more years of experience managing a Medicare Advantage panel of patients
• Associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$255,000 - $298,500 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
