HEALTH SOLUTIONS in Pueblo, CO is seeking a full-time Physician Assistant to join our Family Medicine team at The Family Center. You will provide comprehensive primary care across the lifespan, conduct exams, order tests, prescribe medications, and participate in care coordination within a collaborative medical services team.

The role requires graduation from an accredited PA program, CO license, prescriptive authority, DEA registration, and BLS/CPR.

Qualifications

• Graduate of an accredited Physician Assistant program.
• Current, unrestricted Physician Assistant license in the State of Colorado.
• Prescriptive Authority recognized by the Colorado Medical Board.
• Active DEA registration (or the ability to obtain).
• Current Basic Life Support/CPR Certification.

Responsibilities

• Provide comprehensive primary care across the lifespan, including health assessments, physical exams, diagnostic evaluations, and management of acute and chronic conditions.
• Order, perform, and interpret diagnostic tests to support accurate diagnoses.
• Develop and adjust treatment plans, prescribing medications and preventive interventions.
• Deliver preventive services such as wellness visits, vaccinations, screenings, and routine check-ups.
• Engage patients in shared decision-making and self-management support; educate on prevention and healthy habits.
• Coordinate referrals and care transitions across multiple providers and settings.
