Job summary

Infinite Medical Group is seeking a Primary Care Nurse Practitioner or Physician's Assistant to provide on-site care to residents in long-term care facilities in Uniontown, PA. This part-time role (3 days per week) offers collaborative care in a mission-driven environment with competitive compensation.

The ideal candidate holds an active NP/PA license, has at least 6 months of LTC experience, and can manage chronic and acute conditions while educating patients and families about care options.

Qualifications

• Active and unrestricted NP/PA license to practice in PA.
• State-specific license in good standing in Pennsylvania.
• Minimum 6 months of experience in long-term care or skilled nursing facilities.
• Strong communication, clinical, and critical thinking skills.
• Must be able to lift 20 pounds and stand/walk up to 6–8 hours per day.

Responsibilities
