Cornerstone Clinical Partners in Reidsville, NC, seeks a Primary Care NP/PA to provide on-site primary care for residents in skilled nursing and long-term care facilities. This is a part-time role offering meaningful patient relationships and a collaborative care environment.

You will perform exams, manage chronic and acute conditions, prescribe medications, and coordinate with facility staff to maintain quality of care.

Qualifications

• Active Nurse Practitioner or Physician Assistant license in NC.
• Minimum 6 months of long-term care or skilled nursing facility experience.
• Strong communication, clinical, and critical thinking skills.

Responsibilities

Job Summary

Requirements

• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
• State-specific license in good standing in North Carolina
• Minimum 6 months of experience in long‑term care or a skilled nursing facility
• Strong communication, clinical, and critical thinking skills
• Passion for elder care and person‑centered healthcare
• Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6–8 hours per day
