Job summary

Cornerstone Clinical Partners seeks a Primary Care Nurse Practitioner or Physician Assistant to provide on-site care to residents in long-term care facilities. This full-time role emphasizes meaningful patient relationships and collaborative care in a focused region.

You will perform exams, manage chronic and acute conditions, order tests, develop care plans, and work with interdisciplinary teams to ensure high-quality, patient-centered care for seniors.

Qualifications

• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• North Carolina license in good standing.
• Minimum 6 months of experience in long‑term care or a skilled nursing facility.
• Strong communication, clinical, and critical thinking skills.
• Ability to lift at least 20 pounds and stand/walk up to 6–8 hours per day.
