Job summary

Cornerstone Clinical Partners is seeking a Primary Care Nurse Practitioner/Physician’s Assistant (NP/PA) to deliver on-site care to residents in long-term care facilities in Gastonia, NC. This is a part-time role, two days per week, with salary depending on experience.

You will perform exams, manage chronic and acute conditions, order tests, and collaborate with facility staff to create individualized care plans while providing patient-centered care and ensuring safety and respect for residents.

Qualifications

• Active and unrestricted NP/PA/MD license.
• State-specific NC license in good standing.
• Minimum 6 months of experience in long-term care.
• Strong communication, clinical and critical thinking skills.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
