Fortis Care is seeking a Nurse Practitioner to provide comprehensive care in a single skilled nursing facility. This is a Monday–Friday role with no night or weekend call, designed for clinicians focused on high-quality, patient-centered care within a collaborative healthcare team.

You will assess, diagnose, and manage patients, coordinate with physicians, staff, and families, and ensure timely and effective care with strong documentation and adherence to evidence-based protocols.

What You’ll Be Doing

The Nurse Practitioner (NP) will be responsible for providing comprehensive care to patients in a single skilled nursing facility. This is a Monday–Friday role with no night or weekend call, designed for clinicians who want to focus on delivering high-quality, efficient, and patient-centered care. You will collaborate with the facility staff, physicians, and other members of the healthcare team to ensure timely, effective, and coordinated care.

Key Responsibilities

• Provide direct medical care to patients in the assigned facility, including assessment, diagnosis, treatment, and follow-up.
• Manage acute and chronic conditions, focusing on high-quality, efficient care delivery.
• Coordinate with facility staff, specialists, and families to develop and update individualized care plans.
• Support transitions of care, including post-hospital evaluations, medication reconciliation, and patient/family education.
• Maintain thorough, timely, and compliant documentation of patient encounters and care plans.
• Participate in quality initiatives and process improvements that enhance patient outcomes and efficiency.
• Collaborate with the interdisciplinary team to ensure continuity of care and adherence to evidence-based protocols.

Qualifications

• Current Nurse Practitioner license in the state of Georgia.
• Certification in Internal Medicine, Geriatrics, or Family Practice required.
• Must have an unrestricted DEA license
• Minimum 1–2 years of NP experience preferred; experience in post-acute, SNF, or geriatrics strongly preferred.
• Ability to work independently in a facility setting with physician support.
• Familiarity with EHR systems and clinical documentation best practices.
