Job summary

Fortiscares seeks a Nurse Practitioner to deliver comprehensive care in a single skilled nursing facility in Georgia. This Monday–Friday role focuses on high-quality, patient-centered care with collaboration across the care team.

The NP will manage acute and chronic conditions, coordinate care with families and staff, and maintain thorough documentation. GA NP license and unrestricted DEA are required; 1–2 years SNF/geriatric experience preferred.

Qualifications

• Current Nurse Practitioner license in the state of Georgia.
• Certification in Internal Medicine, Geriatrics, or Family Practice.
• Unrestricted DEA license.
• Minimum 1–2 years NP experience; post-acute/SNF/geriatrics preferred.
• Ability to work independently with physician support.
• Familiarity with EHR systems and clinical documentation practices.

Responsibilities

• Provide direct medical care to patients in the facility, including assessment, diagnosis, treatment, and follow-up.

What You’ll Be Doing

The Nurse Practitioner (NP) will be responsible for providing comprehensive care to patients in a single skilled nursing facility. This is a Monday–Friday role with no night or weekend call, designed for clinicians who want to focus on delivering high-quality, efficient, and patient-centered care. You will collaborate with the facility staff, physicians, and other members of the healthcare team to ensure timely, effective, and coordinated care.

Key Responsibilities

• Provide direct medical care to patients in the assigned facility, including assessment, diagnosis, treatment, and follow-up.
• Manage acute and chronic conditions, focusing on high-quality, efficient care delivery.
• Coordinate with facility staff, specialists, and families to develop and update individualized care plans.
• Support transitions of care, including post-hospital evaluations, medication reconciliation, and patient/family education.
• Maintain thorough, timely, and compliant documentation of patient encounters and care plans.
• Participate in quality initiatives and process improvements that enhance patient outcomes and efficiency.
• Collaborate with the interdisciplinary team to ensure continuity of care and adherence to evidence-based protocols.

• Strong clinical knowledge of adult and geriatric medicine, with expertise in chronic disease management.
