Job summary

Oak Street Health in Indianapolis, IN is seeking a Nurse Practitioner to deliver value-based primary care to adults on Medicare. You will work with a collaborative care team to provide comprehensive care, conduct visits, wellness assessments, and care coordination in an outpatient setting.

Required: MSN/NP doctorate or NP certification, board certification in FNP/AGNP/ACNP/GN, active NP license and DEA, US work authorization; bilingual a plus.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in at least one: Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP or Gerontological NP
• Active, non-probationary NP license
• Active DEA license
• US Work Authorization
• Bilingual proficiency is a plus

Responsibilities

• Provide exceptional primary care for adults on Medicare.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and HRAs.
• Care coordination with other providers and facilities.
• Population health leadership with the care team.
• Phone triage and outreach support.
• Educate patients on conditions, plans and treatments.
• Participate in promotional activities and home visits as needed
