Job summary

Allegheny Health Network is seeking a Nurse Practitioner to provide primary healthcare services for non-acute or chronic conditions in a patient-friendly primary care setting. You will assess, diagnose, treat, and educate patients, coordinate with a care team, and ensure accurate documentation.

Role requires an MSN, NP board certification, and active NP licensure. Prescriptive authority and federal background clearances are required within 60 days of hire; HIPAA and privacy obligations apply.

Qualifications

• Master's degree MSN from the practicing State Board of Nursing Approved Master's Program.
• National Board Certification for Nurse Practitioners through an accredited association.
• Practicing-state licensure as a Nurse Practitioner.

Responsibilities

• Provides appropriate patient care including recording medical history and physical assessment, developing therapeutic care plans, ordering diagnostic tests, interpreting results, and documenting medical records.
• Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team, involving attending physician in plan of care.
• Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the agreement with supervising physician(s).
• Prepares written prescription orders, administers medications/injections, sutures wounds and provides wound care.
• Educates patients and families on health, medications, treatment instructions and community resources.
• Triages patient telephone calls and provides ongoing monitoring and continuity of care.
• Develops and implements performance or quality improvement initiatives and ensures regulatory compliance.
• May perform other duties such as outpatient management and specialty-specific tasks.
