Job summary

Allegheny Health Network seeks a Nurse Practitioner to deliver primary care in a physician office setting, including non-acute and chronic health management. The role includes weekend and on-call coverage as part of rotating duties.

The NP will assess patients, develop care plans, coordinate with the care team, prescribe medications, educate patients, and participate in quality initiatives while adhering to regulatory requirements.

Qualifications

• Master's degree MSN from an approved program.
• National Board Certification for Nurse Practitioners.
• Practicing-state licensure as a Nurse Practitioner.

Responsibilities

• Record patient medical history and perform physical assessments; develop therapeutic care plans.
• Coordinate patient care with patient, family, community health services and interdisciplinary team.
• Prepare written prescription orders, administer medications and injections, and provide wound care.
• Educate patients and family on health, medications, instructions, and community resources.
• Triage patient telephone calls and provide monitoring and continuity of care.
• Support performance and quality improvement initiatives and comply with regulatory requirements.
