Qualifications

• Master's degree MSN from the practicing State Board of Nursing Approved Master's Program.
• National Board Certification for Nurse Practitioners through an accredited association
• Practicing-state licensure as a Nurse Practitioner

Responsibilities

• Provide primary care to non-acute or chronic conditions, including recording medical history and physical assessment and developing therapeutic care plans.
• Coordinate patient care with family, community services and interdisciplinary team, involving supervising physician as needed.
• Monitor continuity of care and coordinate periodic evaluation by physicians.
• Prepare written prescriptions, administer medications, and provide wound care.
• Educate patients and families on health, medications, and preventive care; advise on community resources.
• Triage telephone calls and ensure continuity of care.
• Participate in performance and quality improvement initiatives and ensure regulatory compliance.
• May perform other duties related to outpatient care as assigned.
