Job summary

Mid-Atlantic Physician Group is seeking a Primary Care Nurse Practitioner to provide on-site care to residents in long-term care settings. This full-time role offers meaningful patient relationships and a collaborative care environment in a physician-led group.

The ideal candidate holds an active NP license in North Carolina, with experience in long-term care. You will work closely with interdisciplinary teams to deliver high-quality care while supporting autonomy and professional growth.

Qualifications

• Active and unrestricted Nurse Practitioner license.
• State-specific license in good standing in North Carolina.
• Minimum 6 months of experience in long-term care or skilled nursing facilities.
• Strong communication, clinical, and critical thinking skills.
• Passion for elder care and person-centered healthcare.
• Must be able to lift at least 20 pounds and stand/walk/work on feet 6–8 hours daily.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
