Job summary

Cornerstone Clinical Partners in Clemmons, NC seeks a Primary Care Nurse Practitioner/Physician Assistant to provide on-site care to long-term care residents. This is a full-time position scheduled for four days per week and emphasizes meaningful patient relationships within a collaborative care environment.

Responsibilities include exams, chronic and acute condition management, testing, care-plan development, prescriptions, and teamwork with facility staff to deliver high-quality elder care.

Qualifications

• Active and unrestricted NP/PA license in NC.
• Minimum 6 months in long-term care or skilled nursing facility.
• Excellent communication and critical thinking.
• Ability to lift 20 pounds and stand/walk up to 6–8 hours.

• Perform physical exams and history assessments.

Requirements

• Passion for elder care and person-centered healthcare
• Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day
