Job summary

Community Health Center in Hartford, CT is seeking a part-time Primary Care Nurse (PCN) to deliver high-quality nursing care in a busy primary care setting. The role involves nurse-led visits, care coordination with physicians, patient education, and performance of clinical tasks within licensure.

The PCN will work 32 hours per week, Monday to Thursday 8am–5pm with one Saturday per month 8am–4pm. Preference for BSN/ASN and bilingual candidates; current RN license required; EHR experience

Qualifications

• Registered nurse in good standing and current nursing license.
• BSN or ASN preferred.
• Bilingual ability is preferred.
• Electronic health record experience.
• 1+ year of ambulatory care nursing experience preferred.

Responsibilities

• Provide nursing care under the planned care model and support nurse-led visits.
• Collaborate with PCPs to identify preventive and chronic care needs and coordinate care.
• Educate patients during visits and by telephone on health issues and self-management.
• Assist with exams, treatments, and procedures as needed.
• Maintain infection control and accurate documentation in the medical chart.
