Job summary

Community Health Center in Clinton, CT is seeking a Primary Care Nurse for a 24 hour/week role, including one Saturday per month and one evening per week. Benefits include comprehensive health care, 403B, tuition reimbursement, and paid time off.

The PCN will operate at the top of licensure, provide education, coordinate care with PCPs, and support transitions across care levels within CHCI's chronic care and rapid improvement framework.

Qualifications

• ASN and BSN degrees required or preferred.
• Experience in primary care nursing and clinical teamwork helpful.

Responsibilities

• Support primary care of patients under planned care model.
• Administer medications and vaccines with adherence to orders and protocols.
• Coordinate tests, referrals, and patient education.
• Maintain infection control and accurate documentation per CHCI policies.
