Job summary

Veterans Health Administration, part of the Department of Veterans Affairs, is seeking a Licensed Practical Nurse to join a Patient Aligned Care Team (PACT). The role involves independent nursing tasks under RN supervision, medication administration, and patient education to support comprehensive primary care for Veterans.

The LPN operates within the VA's team-based model, advancing through GS-03 to GS-06 with demonstrated performance and ongoing education, while delivering high-quality nursing

Qualifications

• Active, full, current, and unrestricted LPN/LVN license in the United States
• United States citizenship or eligible per VA policies
• English language proficiency
• Ability to perform nursing duties under RN/MD/DO supervision
• Ability to document and communicate patient information accurately
• Ability to recognize emergencies and initiate appropriate actions
• Demonstrated ability to work within a nursing team and provide patient education

Responsibilities

• Obtain, record, and report vital signs and measurements per standing orders
• Administer medications and document responses according to policy
• Assist with procedures, ensure privacy and comfort
• Provide patient and family health education on self-management and prevention
• Coordinate with RN/MD for appropriate care and escalation
