Job summary

Oak Street Health in Marietta, GA is seeking a Nurse Practitioner to deliver value-based primary care to adults on Medicare. You will see patients independently and collaborate with physicians, focusing on longitudinal care and strong patient relationships.

This full-time role supports a collaborative outpatient care team, with annual wellness visits, care coordination, and opportunities to participate in population health initiatives. Home visits may be required.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program graduate.
• National certification in at least one NP specialty: Family, Adult-Gerontology or Gerontological.
• Active, non-probationary state NP license.
• Active DEA license.
• US work authorization.

Responsibilities

• Provide exceptional primary care to adults on Medicare.
• See patients independently and collaborate with physicians.
• Deliver longitudinal care and build patient relationships.
• Care coordination with other providers, specialists, labs and facilities.
• Population health leadership in coordination with the care team.
• Assist with phone triage and outreach.
• Educate patients on conditions, plans and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.

Education

MSN / DNP / NP certificate

NP certification in Family / Adult-Gerontology / Gerontologic

State NP license active

Active DEA license
