Oak Street Health seeks a Nurse Practitioner to deliver value-based primary care to Medicare patients in our Georgia clinics. You will work independently with physicians on a collaborative care team to keep patients healthy and out of the hospital.

This full-time, Monday–Friday role includes annual wellness visits, care coordination, and population health activities, with a competitive salary and a comprehensive benefits package.

Qualifikationen

• MS in Nursing, DNP, or NP certificate required.
• National certification in one of: Family NP, Adult-Gerontology Primary Care NP, Adult NP, Gerontological NP.
• Active, non-probationary NP license.
• Active DEA license.
• US work authorization.

Aufgaben

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with other providers, specialists, testing facilities, and agencies.
• Lead population health initiatives in coordination with the care team.
