Job summary

Atrius Health, part of the Optum family, seeks a Nurse Practitioner or Physician Assistant to join our Chestnut Hill, MA primary care team. You will provide comprehensive clinical and administrative coordination for a panel of patients.

The role encompasses examinations, diagnoses, treatment, care plan supervision, and coordination with specialists. You will partner with patients and families, supervise staff, and help drive value-based care across the care delivery system.

Qualifications

• Graduate of an Accredited NP program (Masters preferred).
• Licensed by the Massachusetts board of registration in Nursing or for PA by state licensing.
• National board certification.
• Current active Mass DPH CS Registration or ability to obtain post-hire.
• Current active DEA licensure or ability to obtain post-hire.
• Maintains BLS certification, professional certification as appropriate.

Responsibilities

• Comprehensive, longitudinal clinical and administrative coordination of all care for a specific panel of patients.
• Examination, diagnosis and treatment in a primary care setting including supervision of care plan and referrals.
• Oversees monitoring and management of patients with acute and chronic illnesses; acts as care plan author and supervisor.
• Partners with patients and families to manage care plan.
• Provides consultation regarding hospitalized patients in the panel.
• Provides direction and support to staff to assure departmental effectiveness and efficiency.
