Job summary

Crescent Medical Clinic in Eugene, OR, invites a Nurse Practitioner or Physician Assistant to join a collaborative primary care team. You will work alongside physicians, pharmacists, nurses, care managers, and behavioral health professionals to provide high-quality care with reduced administrative burden.

The role features an outpatient setting, a four-day work week for full-time APCs with hybrid options, and strong multidisciplinary support, including an onsite pharmacist and RN case manager.

Qualifications

• Master's or Doctoral degree from an accredited Nurse Practitioner or Physician Assistant program.
• Board certification as a NP/PA or ability to obtain prior to start.
• Oregon license or ability to obtain prior to start; DEA registration or ability to obtain prior to start.

Responsibilities

• Outpatient primary care with four-day work week for full-time APCs; hybrid roles for eligible candidates.
• Partner with physicians in a shared-panel, team-based care model.
• No call; protected administrative time and structured onboarding.
• Opportunity to perform office-based procedures based on experience.
• Onsite lab and imaging services with Epic EMR and AI-powered documentation support.
