Bookmark Medical in White House, TN, is seeking a Primary Care Advanced Practitioner (NP/PA) to join our established Primary Care team. This full-time role offers a base salary in the range of $115,000 to $135,000 plus incentives, with a focus on patient-centered care and collaboration.

You're expected to provide comprehensive care, perform exams, educate patients, and coordinate with specialists. A strong EMR background is preferred, along with NP/PA certification and licensure.

Qualifications

• Graduate of an accredited NP or PA program; current certification and licensure.
• At least one year of APP experience, preferably in primary care.
• Strong clinical skills, collaborative mindset, and proficiency with EMR systems.

Responsibilities

• Provide comprehensive care to a diverse patient population.
• Conduct thorough physical exams, diagnose medical conditions, and develop personalized treatment plans.
• Promote health education and disease prevention.
• Order and interpret diagnostic tests; manage chronic conditions.
• Collaborate with specialists and other healthcare providers to ensure coordinated care.
• Maintain accurate up-to-date medical records and patient charts.
• Stay current with medical advancements and best practices in Primary Care.
