Fast Pace Health in Tennessee is seeking a Primary Care Provider: Nurse Practitioner or Physician Assistant to deliver preventive and chronic care in a team-oriented clinic serving rural communities. The role emphasizes patient-centered care, collaboration with physicians, and staying current with medical advances.

Ideal candidates hold a Master’s degree (Nurse Practitioner or PA) and have at least 1 year of direct primary care experience.

Qualifications

• Master’s degree in Nursing (MSN) or Master’s degree in Physician Assistant Studies (MMS).
• 1 year of dedicated experience in direct primary care settings as a provider (NP/PA).
• Proficiency in chronic care management, preventive health measures and annual wellness visits.

Responsabilités

• Provide comprehensive primary care including chronic disease management and preventive services.
• Prescribe medications and develop patient care plans in collaboration with physicians.
• Engage in patient education, health literacy, and shared decision-making.
• Participate in telehealth and on-site clinic operations to ensure high-quality patient care.
