Hometown Urgent Care & Occupational Health in Springfield, OH is seeking a compassionate Primary Care Provider/Advanced Practice Provider (NP or PA) to deliver comprehensive care across the lifespan in a team-based outpatient setting.

License in Ohio required (NP-C/FNP or PA-C). Experience in primary care, strong communication and empathy, and EMR proficiency (E-ClinicalWorks) are essential. The role offers a collaborative environment, ongoing learning, and a competitive benefits package.

Qualifications

• Licensed in Ohio as NP-C/FNP or PA-C.
• Experience in primary care, family medicine, or internal medicine.
• Strong communication and empathy; ability to connect with patients.
• Experience with outpatient clinical procedures.
• Commitment to teamwork and continuous learning.
• E-ClinicalWorks EMR experience is a plus.

Responsibilities

• Conduct comprehensive physical exams, assessments, and patient histories.
• Diagnose and manage a wide range of conditions.
• Provide primary care support across multiple specialties.
• Perform in-scope procedures.
• Collaborate with ENT, Surgical, and Medical Subspecialties for continuity of care.
• Utilize E-ClinicalWorks for documentation.
• Provide emergency and urgent interventions when necessary.
• Educate patients and families on treatment plans and prevention.
