Job summary

The NP will provide care across the lifespan, including preventive services, chronic disease management, and telehealth, collaborating with physicians, behavioral health, and community

Qualifications

• Master’s or Doctorate degree in Nursing (NP or DNP).
• Active NY NP license and prescriptive authority.
• National board certification (FNP/AGNP/PNP).
• Current DEA registration and BLS/CPR certification.
• Clinical experience or interest in underserved populations.
• Must possess New York State Medicaid identification.
• Meet New York State's 3,600-hour clinical experience requirement for independent NP practice.

Responsibilities

• Provide comprehensive primary care across the lifespan.
• Perform health assessments, diagnose conditions, develop and implement treatment plans.
• Manage chronic illnesses within NP scope.
• Provide preventive services and patient counseling.
• Order, perform, and interpret diagnostic tests.
• Prescribe medications per state regulations.
• Provide care via in-person visits, telehealth, and outreach.
