Job summary

Joseph P Addabbo Family Health Center Inc seeks a Nurse Practitioner to deliver high-quality, patient-centered primary care. You will focus on health promotion, disease prevention, chronic disease management, and patient education in a diverse, underserved population.

Collaborating with an integrated care team, you will provide in-person and telehealth services, document thoroughly in the EHR, and participate in quality improvement initiatives.

Qualifications

• Master’s or Doctorate in Nursing (NP or DNP).
• Active NY NP license with prescriptive authority.
• National board certification (FNP/AGNP/PNP).
• Current DEA registration (if applicable) and BLS/CPR.
• Experience or strong interest in caring for underserved populations.
• New York State Medicaid identification and 3600-hour clinical requirement.

Responsibilities

• Provide comprehensive primary care across the lifespan.
• Manage chronic illnesses (diabetes, hypertension, asthma, COPD, behavioral health).
• Provide preventive services, screenings, immunizations, and counseling.
• Collaborate with physicians, nurses, behavioral health, and care teams.
• Document encounters in the EHR and ensure regulatory compliance.
