Qualifications

• Licensed Physician (MD/DO) in New York State.
• Board-certified or board-eligible in Family Medicine or Internal Medicine.
• Current DEA registration; ability to obtain X-Waiver/MAT if required.
• Current BLS/CPR certification.
• Experience or strong interest in working with underserved populations.

Responsabilités

• Deliver comprehensive, patient-focused primary care for adults.
• Perform assessments, diagnose, treat, and follow-up per guidelines.
• Manage acute and chronic conditions (HTN, diabetes, asthma, COPD, behavioral health).
• Provide preventive care (annual physicals, immunizations, screenings, health education).
• Order/interpret tests; arrange referrals as needed.
• Deliver care in-person and via telehealth; participate in on-call coverage.
• Document encounters in EHR timely and accurately.
• Collaborate with multidisciplinary team; participate in care planning and quality improvement.
• Coordinate transitions, referrals, and follow-ups; communication with specialists and community resources.
