Trinity Health Mid-Atlantic Medical Group in Pennsylvania is seeking a Primary Care APP to support expanded access and high-quality care across the lifespan. This full-time role offers integrated Epic EMR and a collaborative team environment.
You will provide comprehensive primary care, manage acute and chronic conditions, emphasize prevention, document timely, and participate in quality improvement initiatives to enhance community health.
Formación
- Nurse Practitioner license from an accredited program.
- Eligible for Pennsylvania licensure.
- Current DEA license or ability to obtain before start date.
- Current BLS certification required.
- Commitment to high-quality, patient-centered care.
Responsabilidades
- Provide comprehensive primary care for patients across the lifespan.
- Assist with acute and chronic diagnosis and management of conditions.
- Emphasize preventive care and population health management.
- Collaborate with interdisciplinary teams to optimize patient outcomes.
- Maintain timely and accurate documentation.
- Participate in quality and performance improvement initiatives.
