Oak Street Health is seeking a Nurse Practitioner to deliver value-based primary care to adults on Medicare, operating in outpatient clinics with a collaborative care team. You will see patients independently, coordinate with physicians and other professionals, and participate in population health initiatives to improve outcomes.

The role emphasizes comprehensive wellness visits, patient education, and coordination with specialists, with a strong focus on teamwork and community health impact.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in Family NP, Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Responsabilités

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and HRAs with a holistic health focus.
• Care coordination with providers, specialists and testing facilities.
• Population health leadership with the Care Team, including cancer screening reminders.
• Assist the care team with phone triage and outreach.
• Educate patients on health conditions, care plans and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties as assigned.
