TriHealth is seeking a Licensed Practical Nurse to provide direct and indirect patient care in a primary care office in Blue Ash, OH. The Medical Home LPN coordinates pre-visit planning, communicates with patients through MyChart, and supports care transitions as part of a physician-led team.

Ideal candidates have BLS/CPR, EMR and billing knowledge, and 3–4 years of clinical healthcare experience in a physician practice or related field.

Qualifications

• Graduate of an approved healthcare program.
• BLS and CPR certification required.
• 3–4 years clinical healthcare experience in a physician practice or related field preferred.
• Knowledge of EMR and billing processes helpful.

Responsabilités

• Coordinate primary care rooming and procedures for adults and children.
• Document care details and manage patient messages in MyChart.
• Support pre-visit planning and care coordination.
• Aid in transitions of care and follow-up referrals.
• Participate in wellness and prevention initiatives.
