Job summary

OnePeak Medical Providers is seeking a skilled Medical Provider (PA-C) to join our growing team in a comprehensive primary care model focused on root-cause, functional medicine. The role emphasizes patient relationships, education, and collaborative care to optimize patient outcomes.

Qualifications

• Active Oregon PA license or eligibility to obtain.
• Board certification in Family Medicine or Internal Medicine is required.
• Strong interest in preventive and functional medicine.
• Excellent communication and patient relationship skills.
• Able to work in a fast-paced, collaborative environment.
• Proficient in using electronic health records (NextGen experience is a plus).

Responsibilities

• Provide direct patient care through office visits and telehealth.
• Order/interpret labs and imaging; diagnose acute and chronic conditions.
• Develop personalized treatment plans including lifestyle interventions and therapies.
• Educate patients on preventive health and functional medicine concepts.
• Collaborate with health coaches, nutritionists, mental health providers, and staff.
• Document visits in NextGen EHR timely and compliant.
• Participate in team meetings and quality improvement initiatives.

JOB SUMMARY:

We are seeking a passionate and skilled Medical Provider to join our growing team. RESPONSIBILITIES AND DUTIES:

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR (NextGen) in a timely and compliant manner

Participate in team meetings, quality improvement initiatives, and training sessions

Support clinic operations by contributing to a positive, team-oriented environment

REQUIRED QUALIFICATIONS

• Active Oregon license as a Certified Physician Assistant (or eligibility to obtain)
• Board certification in Family Medicine or Internal Medicine
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
• Proficient in using electronic health records (NextGen experience is a plus)
