Job summary

Hartford HealthCare Medical Group is seeking a Full-time Family Nurse Practitioner or Physician Assistant for a multi-provider, primary care practice seeing patients age 12+. The position is based in Plainfield and Putnam, CT.

Open to Family Nurse Practitioners (FNP) and Physician Assistants (PA). Schedule features 4 days per week (3 in Plainfield, 1 in Putnam) with no call and extensive benefits.

Qualifications

• Licensure: Unrestricted State of Connecticut Medical License (or eligible).
• Training: APRN (Family Nurse Practitioner) or PA.
• Experience: Two years of post-graduate experience as a Nurse Practitioner or Physician Assistant in Primary Care or related field seeing both peds and adults.

Responsibilities

• Provide primary care services in an outpatient setting for patients aged 12+.
• Perform routine physical examinations and develop medical, educational, and psychosocial treatment plans.
• Collaborate with physicians and care teams to coordinate patient care.
