Job summary

BAS Healthcare, Inc invites qualified Advanced Practitioners to join an outpatient primary care team serving patients aged 12 and up across Plainfield and Putnam, CT. The role offers a 4-day week (3 days Plainfield, 1 day Putnam) with no on-call requirement and a strong support system.

Unrestricted CT license or eligibility is required; FNP or PA training is necessary, with 2+ years of post-graduate primary care experience preferred. Employed salary with benefits is offered.

Qualifications

• Unrestricted State of Connecticut medical license required (or eligible).
• APRN (Family Nurse Practitioner) or PA training required.
• Two or more years of post-graduate experience in Primary Care or related field preferred.

Responsibilities

• Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients.
• Develop plans for further evaluation including referrals to specialists and other health team members.
• Provide appropriate treatment within scope of practice and refer when needed.
• Work under established guidelines in collaboration with practice physicians.
