Responsibilities

• Provide comprehensive, evidence-based primary care to patients 18 months and older.
• Perform assessments, diagnose acute and chronic conditions, and develop treatment plans.
• Prescribe medications and order/interpret diagnostic tests per state licensure.
• Manage chronic conditions including hypertension, diabetes, and asthma.
• Deliver preventive care services including vaccinations and routine physicals.
• Document encounters in EMR and coordinate with interdisciplinary teams.
