Qualifications include graduation from an accredited practical nursing program and active LPN licensure in the state of practice; 6 months of primary care experience is preferred.
Job Profile Summary
Under the direction of the primary care operations team, the Primary Care Patient Care LPN works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Patient Care LPN will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for all patients.
