Mercy operates the Clinic Primary Care – Patients First Drive in Washington, MO. The role supports Mercy Primary Care Providers to deliver safe, timely, and patient-centered care, partnering with patients and the care team to drive quality outcomes for an attributed panel.

Days are Monday–Friday, with a hybrid on-site/work-from-home schedule after an initial on-site training period. LPNs and RNs are both sought, with on-site training followed by a 50/50 hybrid model and monthly on-site clinic

Qualifications

• LPN must hold a Diploma in Practical Nursing and an active LPN license.
• RN must have an Associate of Applied Science in RN, with active RN license.
• 2–3 years of relevant clinical experience preferred for either track.

Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards.

Schedule

Monday – Friday, 0800-1630

No nights, weekends, or holidays!

Hybrid Schedule Requirements

• Must complete three months of on-site training prior to transitioning to a hybrid work-from-home schedule. After training, this role follows a 50/50 hybrid model, with full-time (1.0 FTE) co-workers expected to work on-site five days per pay period. Must also be available to attend monthly on-site clinic meetings.

Highly Preferred Skills

• Proficiency with Epic EMR system.
• Timely and thorough patient triage skills.
• Computer competency (most of this position will be computer work).
• Excellent time management and prioritization.
• Adaptability in a new and changing environment.

LPN Qualifications

• Education: Diploma in Practical Nursing.
• Licensure: Active LPN license.
• Experience: 2-3 years of experience preferred.

RN Qualifications

• Education: Associate of Applied Science in Registered Nursing, Graduation from registered nursing program, bachelor's degree in nursing (BSN), OR 4 years of commensurate experience in lieu of the bachelor’s degree.
• Licensure: Active RN license.
• Experience: 2-3 years of experience preferred.
