The APP will perform exams, create care plans, order tests, write prescriptions, and coordinate with home health and other services to ensure patient-centered care. Florida license required.

Qualifications

• Ability to perform comprehensive patient assessments and develop care plans.
• Strong communication with patients, families, and care team.
• Proficient in electronic medical records and CMS guidelines.

Responsabilités

• Provide total care for assigned patients in collaboration with physician and team.
• Document all care in EMR and follow CMS policies.
• Order tests, treatments, and home health as indicated.
• Counsel patients on therapeutic regimens and follow-up care.
• Coordinate with home care and community resources as needed.
