Mosaic Health, LLC. is seeking an Advanced Practice Provider (APRN or PA) for primary care in Florida.

You will deliver total patient care in collaboration with the physician of record and patient care team, assess patients, develop care plans, manage prescriptions, and coordinate home health and therapies as indicated. You will also focus on quality measures and HCCs, ensuring timely documentation and billing while consulting the supervising physician on complex cases.

Qualifications

• Licensed Florida APRN or PA in good standing
• Ability to communicate across care teams and with families
• Experience in acute, chronic and palliative care

Responsabilités

• Provide total patient care in collaboration with the physician of record and care team
• Perform patient assessments/exams and develop care plans
• Diagnose, order labs/imaging, write prescriptions and provide interventions as indicated
• Coordinate with home health, therapies and DME as needed
• Address quality measures and HCCs to meet/exceed goals
• Complete documentation and billing in a timely manner; consult supervising physician for complex cases
