Qualifications

Responsibilities

• Provide total care for assigned patients with the care team.
• Assess patients, plan care, and manage acute and chronic conditions.
• Diagnose and decide treatment; interpret tests and imaging as needed.
• Document in EHR per policy and CMS requirements.
• Lead nursing process and coordinate program-level improvements.
• Refer patients to home health, therapies, and DME as needed.
• Counsel on treatment adherence and end-of-life planning.
