Job summary

Millennium Physician Group is seeking an Advanced Practice Provider (APRN or PA) to deliver total primary care in collaboration with the physician and care team. The APP will assess, plan, treat, and document across acute, chronic, and palliative needs, with a Florida license in good standing.

You will interpret tests, manage care plans, and communicate with other providers to ensure patient-centered outcomes, including end-of-life planning when appropriate.

Qualifications

• Licensed APRN or PA in good standing (FL).
• Experience in primary care/family practice beneficial.
• Ability to manage acute, chronic, and palliative care within scope.

Responsibilities

• Provide total patient care in collaboration with physician and care team.
• Perform assessments, create care plans, and order/interprete tests.
• Document timely and coordinate with care providers for continuity.
