Frederick Health Medical Group (FHMG) in Maryland is seeking an experienced Nurse Practitioner or Physician Assistant to join our Primary Care team in a full-time role. You will provide comprehensive evaluation and management for adult and geriatric patients.

This Advanced Practice Provider will collaborate with physicians, nursing staff, and care coordinators to ensure high-quality, patient-centered care and contribute to population health initiatives and quality improvement in a physician-led

Qualifications

• Master’s degree from an accredited nursing program or PA program.
• Active Maryland license or ability to obtain.
• Current DEA license and Maryland CDS license (or ability to obtain).
• Current BLS certification (or ability to obtain).
• Unrestricted license with no history of suspension or revocation.

Responsibilities

• Provide high-quality care to adult and geriatric patients including history taking, exams, diagnosis, treatment planning, and patient education.
• Order and interpret labs, imaging, and referrals; perform procedures within scope.
• Collaborate with physicians, nursing staff, care coordinators, and specialists for coordinated care.
• Support care coordination including follow-ups on results and chronic disease management.
• Document accurately and timely in the EMR with appropriate coding and compliance.
• Participate in quality improvement initiatives and population health efforts.
• Maintain professionalism and regulatory compliance, and participate in provider trainings.
