Job summary

Bayview Physicians Group is seeking an Advanced Practice Provider (NP or PA) to join our primary care team in coastal Virginia. You will provide comprehensive, longitudinal patient-centered care in an outpatient setting, collaborating with a multidisciplinary team of 300+ physicians and APPs.

The role includes performing exams, diagnosing and managing common conditions, developing care plans, ordering tests, prescribing medications, and coordinating transitions of care for adults and geriatrics.

Responsibilities

• Perform comprehensive patient evaluations and physical examinations
• Diagnose and manage common acute illness and chronic conditions with focus on hypertension, diabetes, asthma and other primary care issues
• Develop, implement and evaluate individualized care plans
• Order, interpret, and follow up on labs, imaging, and other diagnostic tests
• Prescribe medications and treatments in line with current guidelines
• Provide age-appropriate care for adults and geriatrics (and pediatrics if capable)
• Coordinate transitions of care including hospital follow-ups and post-discharge management
• Collaborate with Bayview specialists through an integrated internal referral network
• Manage daily patient load (~18-20 patients/day)
• Work and collaborate with other Physicians and APPs within the practice

In this Role:
