Job summary

Bayview Physicians Group in Coastal Virginia seeks a Primary Care Physician to join a large, physician-led multi-specialty network. The role centers on outpatient, longitudinal care with a multidisciplinary team and a commitment to value-based care.

You will perform comprehensive evaluations, manage acute and chronic conditions, and coordinate with specialists within an extensive referral network. A competitive compensation package and benefits are offered.

Responsibilities

• Perform comprehensive patient evaluations, medical histories, and physical examinations
• Diagnose and manage a broad range of acute illnesses and chronic medical conditions across a diverse patient population
• Develop, implement, and monitor individualized treatment plans with emphasis on preventive care and chronic disease management
• Order, interpret and follow up on laboratory studies, diagnostic imaging, and other tests
• Prescribe medications and evidence-based treatments
• Provide comprehensive care for adult and geriatric patients (with potential pediatric patient involvement)
• Coordinate transitions of care, including hospital follow-up and post-discharge management
• Collaborate with a network of Bayview specialists through a seamless internal referral system
• See approximately 20 patients per day
• Partner with physicians and advanced practice providers to deliver coordinated, high-quality care

In this Role:

• Prescribe medications and evidence-based treatments consistent with current clinical guidelines
• Provide comprehensive are for adult and geriatric patients (with the opportunity to include pediatric patients based on training and interest)
• Coordinate transitions of care, including hospital follow-up and post-discharge management
• Collaborate with an extensive network of Bayview specialist through a seamless internal referral system
