Job summary

Lake County Government seeks a Prevention Team Supervisor and CMP Coordinator to work with the Human Services Director and IOG, planning, implementing, and coordinating prevention programs for children, youth, families, and school communities.

This role emphasizes data management, collaborative partner engagement, supervision of the Family Support Specialist and the Bilingual Community Caseworker, budget oversight, and ensuring services are delivered in a cohesive, outcome-driven manner.

Qualifications

• Bachelor's degree in social work or a relevant social/human behavioral science program is required.
• Facilitation training or completion within six (6) months of hire.
• At least two years of supervisory experience is required.
• Bilingual in English and in Spanish is preferred.

Responsibilities

• Participate in the development and implementation of the strategic plan, program evaluation, and ongoing communication to meet program objectives.
• Prepare minutes and agendas to assist with the facilitation of IOG Board Meetings.
• Provide opportunities for the IOG Board to review and analyze CMP data.
• Manage the data system, including data collection, data entry, analyses, and reporting.
• Enhance collaboration between stakeholders and serve as a liaison with community agencies; develop relationships with partners.
• Prepare the annual CMP Memorandum of Understanding (MOU) and coordinate signatures.
• Report on updates at the state level and their impact on local CMP requirements.
• Attend State Steering Committee Meetings and State Evaluation efforts.
• Prepare annual reports regarding programming.
• Oversee development, implementation, and data collection of program practices and procedures.
• Assist Director and Deputy Director of Finance in monitoring expenses and budget status.
